ManagerDuties:
- Staff members meet with every client on their caseload
Face-To-Face approximately 2-3 times weekly to provide information
and referral, agency linkage, advocacy, home visits, and follow-up
services to CTT clients as defined by the Treatment Plan and MCO
Authorization request.
- Staff members provide crisis intervention and emergency
services, as well as utilizes Center-wide and community resources
as needed for client benefit.
- Staff members provide transportation to clients as needed to
meet the goals of their Treatment Plan.JOB PURPOSE/SUMMARYSummary
of role of team:
- The Anderson County Continuous Treatment Team (CTT) program, by
virtue of the person its serves, represents a place where the
Center's vision is lived out on a day-to-day basis.
- The severe and/or persistently mentally ill (SPMI) population
truly embodies the poorest of the poor and the sickest of the
sick.
- Therefore, it is the philosophy of the Anderson County CTT
program to live up to this vision by reaching out to the SPMI
regardless of what situation they are in or where they are and by
opening a door of stability through mental health treatment.
- Mental health case managers assist in ensuring that the
consumer/natural support is able to access both cost effective and
quality services.
- Mental health case management requires the mental health case
manager and consumer/natural support to have a strong, productive
relationship, which includes viewing the consumer/natural support
as a responsible partner in identifying and obtaining the necessary
services and resources.
- CTT is defined as a series of actions taken by a specified
mental health CTT team or a specified CTT case manager to assess
needs, plan, coordinate, monitor, adjust, and advocate for consumer
services, treatment, and supports.
- CTT services may support clinical services.
- The case CTT process is directed toward recovery and
self-management of mental illness and provides the consumer/family
opportunity to improve their quality of life. Summary of position:
- Mental health case management is primarily provided in the
community (it is not office based) in settings that are accessible
and comfortable to the consumer/natural support; frequency of
contact is based on clinical need.
- Cultural and ethnic factors should be taken into consideration
in the delivery of these services.
- The services delivered by CTT staff are not time-limited but
are designed according to the needs of the individual being
served.
- Each person receiving CTT will have one primary mental health
case manager even though a mental health CTT team may be
provided.
- Services include identifying the needs and strengths of the
individual, developing a CTT Service Plan with the individual that
identifies the long-term and short-term goals, and assisting the
individual to access these services.
- Staff members are expected to provide information and referral,
agency linkage, advocacy, home visits, and follow-up services to
CTT clients as defined by the Treatment Plan and MCO
authorization.
- Staff members provide crisis intervention and emergency
services, as well as utilizes Center-wide and community resources
as needed for client benefit.

- This position serves adults and children.TYPICAL WORKING
CONDITIONS/ENVIRONMENT
- This position is a community-based position.
- Staff members are provided with all equipment necessary to
remain mobile and be able to work from the community or in their
personal vehicles.
- Staff members do have access to office space on-site and
meeting space for office visits if clients prefer this method of
follow-up and outreach.
- Staff members participate in Treatment Team and Team Meetings
as outlined by their supervisor upon hire.This job description is

- Starting salary for this position is approximately $18.78 /hr
based on relevant experience and education.Schedule:
- This position has a regular schedule of Monday - Friday 8am -
5pm.
- There is no on-call for this position and employee is off on
Center designated holidays.
- In person daily attendance is essential for this position
except in instances of approved time off. Travel:
- This position requires utilizing a personal dependable vehicle
to conduct Center business.
- Maintaining a dependable vehicle and certified driver status is
a condition of employment.
- Staff members will be required to provide transportation in
their personal vehicle and staff members are reimbursed for use of
their vehicle for this purpose.Equipment/Technology:
- This position does require basic computer skills for
timekeeping and use of electronic medical records entry.
- Use of a mileage application on a Center issued phone is also
required for mileage reimbursement.QUALIFICATIONS - Anderson Co.
CTT Case Manager Experience / Knowledge:
- One-year experience in the social work field or a behavioral
health setting preferred. Education / License:
- Must have at least a Bachelor's degree in a health-related
field of counseling, psychology, social work, sociology, or a
behavioral science field with course work or experience in the
areas of cultural diversity, human development, etiology and
treatment of mental illness, alcohol and drug abuse, physical and
sexual abuse, suicide, and mental disabilities.
- Candidates with a Bachelor's degree in rehabilitation,
occupational therapy, criminal justice, or education, must have
fifteen college-level semester hours of coursework in behavioral
health and at least one year of work experience in the behavioral
health setting. Physical/Emotional/Social - Skills/Abilities:
- Exposure to biological hazards.
- Hearing of normal and soft tones.
- Close eye work.
- Valid driver's license- F endorsement.
- Lifting up to 50 lbs.
- Pushing/pulling up to 150 lbs.
- Frequent sitting, standing, walking, bending, stooping, and
reaching.
- Required to be certified in, and effectively implement, verbal
and physical de-escalation techniques that include a wide range of
bodily movements; including but not limited to, grasping, holding
another person, going down on knees, running, and walking.
